- Novo Nordisk’s ziltivekimab did not reduce cardiovascular events or deaths compared with placebo, despite strong expectations that it would.
- The result challenges the “inflammation causes heart disease” hypothesis: Atherosclerosis is strongly associated with inflammation, but this trial raises the possibility that inflammation may sometimes be more of a marker of disease than a direct therapeutic target.
- The ZEUS trial was substantial: More than 6,300 high-risk patients with cardiovascular disease or advanced kidney disease were enrolled, specifically selecting people with elevated inflammatory markers.
- The drug clearly suppressed inflammation—but outcomes did not improve: Ziltivekimab dramatically lowered IL-6 and CRP, yet this biological effect did not translate into fewer heart attacks, cardiovascular complications, or deaths
- This is not the first disappointing anti-inflammatory trial: Colchicine trials have produced mixed results, including a large study of more than 7,000 patients in which CRP fell but cardiovascular outcomes did not improve. An earlier drug, canakinumab, produced modest cardiovascular benefit but increased serious/fatal infections.
- Experts are not ready to abandon inflammation completely: Pathology, animal studies, epidemiologic evidence and some prior clinical trials still support a causal role. The failure could mean ziltivekimab targeted the wrong inflammatory pathway, the patients were too advanced in their disease, or inflammation needs to be treated earlier.
- The key emerging question: Rather than asking simply whether inflammation causes atherosclerosis, researchers may now need to ask which inflammatory pathways matter, in which patients, and at what stage of disease. For now, proven therapies—especially aggressive LDL lowering and traditional risk-factor control—remain the foundation of cardiovascular prevention
